This is what your computer screen looks like when you use the APT Assistant. When you begin to create a new chart, a single large box appears at the top of the screen. As soon as you have completed the naming of this box one or more boxes will automatically appear below. If you click your mouse on the small diamond-shaped icon that is located on the bottom left-hand corner of each box, one or more boxes will appear below this box. 

To add or edit action plan details on any plan chart box, you simply click on the small square icon as illustrated above. A separate window appears to prompt and coach you through the Action Plan creation sequence.
